Unpacking a Typical Autism Study: How Design, Bias, and Assumptions Impact Autism Research
“Get Ready with Me” (GRWM) videos have taken over social media, inviting viewers into the mundane and glamorous alike. But what if we applied this trend to a more intellectual pursuit—analysing autism research? Let’s unpack a study together, scrutinising its design, sample size, reliability, validity, and generalisability, much like getting ready involves examining each part of the process before stepping out.
Research on autistic people often carries the weight of the medical model, framing autism as a problem to be solved, with interventions aimed at “fixing” perceived deficits. This lens permeates the study we’ll explore today, where a proposed Emotional Recognition Memory Training Program (ERMTP) seeks to address “emotional arousal” and “cognitive empathy” in autistic children. The authors describe these as deficits requiring correction, rather than acknowledging them as natural variations in human experience shaped by context, relationships, and power dynamics—an omission typical of medicalised approaches.
Using the Power Threat Meaning Framework (PTMF) and critical theory, we’ll reframe this study, shifting the narrative from pathology to understanding. The small-scale, early-stage design is a chance to question not just the study’s structure but also the assumptions underpinning it. How valid are its claims? Finally, phase three pilot-tested the program with five autistic children and five children who were not autistic. These phases might seem methodical, but the design carries assumptions and limitations that demand scrutiny.
This study took place in Thailand, a country where cultural norms and legal frameworks shape perceptions of autism. Thailand has made strides in recognising autistic individuals, but challenges remain. Services are often patchy, and cultural expectations can pressure parents to present their child’s progress positively. Such pressures may skew parent-reported outcomes, a key measure in this study. First, there are no randomised trials or comparison groups, making it impossible to isolate the program’s true effect from external factors. The reliance on parent reports, particularly in a cultural context where face-saving and parental pride are significant, raises questions about the objectivity of outcomes. Additionally, the inclusion of non-autistic children alongside autistic children in the pilot phase creates a heterogenous sample but does not control for differences that could impact results.
These design choices limit the study’s capacity to draw robust conclusions. The absence of a control group and randomisation means the reported improvements might reflect natural developmental changes or placebo effects rather than the program itself. Parent-reported outcomes, whilst useful for capturing lived experience, must be interpreted cautiously within the socio-cultural dynamics of Thailand.
Ultimately, whilst the design offers insights into the potential feasibility of ERMTP, it tells us more about the study’s framing of autism than the program’s true effectiveness. It underscores how research design, shaped by cultural and systemic influences, affects the narratives we construct about autistic lives. By critically assessing these choices, we can better understand what the study reveals—and what remains obscured.
Sample Size: Small but Mighty or Too Limited?
The ERMTP study’s sample size is undeniably small: five autistic children and five children from the neuro-majority formed the participant groups. Additionally, five ‘experts’ were consulted for content validation. Whilst small samples are not uncommon in pilot studies, their limitations significantly affect the conclusions we can draw from the research.

Variability within the sample can disproportionately influence results, leading to findings that may not generalise beyond the immediate participants. For example, the five autistic children in this study ranged in age from 6 to 8 and were diagnosed with ‘mild-to-moderate autism.’ This excludes children with more significant support needs or those who might process information differently (like me, a gestalt processor), limiting the study’s representativeness. Similarly, the experts involved in content validation—whilst qualified—represent only a narrow perspective on the program’s appropriateness and effectiveness.
In the broader context of Thailand, these limitations intersect with cultural factors. Thailand’s approach to autism, like many countries, is shaped by societal norms that prioritise conformity and the idea of “overcoming” neurodivergence. Families often face societal pressure to ensure their children align with these norms, and interventions like ERMTP are framed as tools to help autistic children “fit in.” In this study, the feedback provided by both parents and participants may reflect these cultural expectations rather than the true outcomes of the intervention.
The program’s focus on recognising and responding to facial expressions raises another concern. Whilst this might be framed as a tool to improve social cognition, it risks encouraging masking—a behaviour where autistic individuals suppress or hide their authentic selves to appear like authentic members of the neuro-majority. Masking can have long-term negative effects, including increased stress, anxiety, and a diminished sense of identity. Is the ERMTP subtly training autistic children to prioritise arbitrary standards of interaction at the expense of their own comfort and authenticity? This question becomes even more critical when considering the feedback nature of the intervention, which might reward certain behaviours without considering the child’s perspective.
That said, small samples are often a necessary starting point in resource-limited settings. Thailand may lack the funding or infrastructure for larger-scale autism research, and studies like this provide a foundation for further exploration. However, the limitations inherent in small sample sizes require caution. This measure would ensure that the five experts validating the program provided consistent evaluations independent of individual bias. Unfortunately, the study does not report inter-rater reliability, leaving us uncertain whether the perfect agreement among the raters reflects genuine consensus or shared assumptions. Without this assurance, the reliability of the study’s conclusions is fundamentally weakened, undermining confidence in the program’s consistency.
The study’s validity also warrants scrutiny. It boasts perfect Item Objective Congruence (IOC) scores of 1.0 across all activity items, suggesting strong alignment with the program’s stated goals. However, IOC only measures how well a task matches its intended purpose, not whether it is effective or reliable in diverse, real-world contexts. To illustrate, consider the analogy of target practice. A shooter might consistently hit the lower left-hand side of the six-ring, indicating reliability but flawed aim, potentially caused by poor breath control or an imprecise trigger pull. This reliability does not equate to accuracy, as the goal is to consistently hit the 10-ring. Similarly, whilst the ERMTP tasks may align perfectly with their stated objectives, the study provides no evidence that they address or assess the natural and authentic emotional recognition or perception of autistic children as perfectly normal in their own right, rather than as flawed members of the neuro-majority.
Adding to these concerns is the study’s reliance on parent-reported outcomes, which introduces a significant source of bias. Parents, often hopeful that interventions will “cure” their child or help them conform to societal norms, may unintentionally over-report improvements. In a cultural context like Thailand, where family pride and societal expectations can heavily influence reporting, these biases may become even more pronounced. The risk here is that the reported outcomes reflect parental expectations, shaped by the desire for their children to conform to societal norms, rather than genuine changes in the children’s natural abilities or behaviours, further calling the findings into question.
Whilst the study proclaims itself valid—boasting perfect IOC scores and glowing parent feedback—its deeper methodological flaws cast significant doubt on this claim. The absence of inter-rater reliability, which would provide critical confirmation of consistent expert evaluations, weakens the credibility of the results. Moreover, the perfect IOC scores themselves, whilst superficially impressive, strain plausibility. In rigorous research, minor disagreements among raters are expected, reflecting the diversity of expertise and critical thought necessary for a robust validation process. The lack of variability suggests either an uncritical evaluation or overly simplistic criteria, which undermines the claim of thorough and meaningful validation.
Additionally, the limited scope of IOC as a validity measure must be addressed. Whilst IOC measures alignment between tasks and study objectives, it does not assess whether the program is effective, reliable, or applicable in real-world contexts. This gap leaves the study unable to demonstrate that its tasks truly achieve the broader aim of enhancing ‘emotional recognition’ in autistic children, particularly when their natural, authentic emotional perception remains unexamined. Instead, the study implicitly frames autistic children as needing to meet the arbitrary standards of the neuro-majority in Thai society, perpetuating a deficit-based view that fails to celebrate their inherent strengths and perspectives.
Parent-reported outcomes further muddy the waters. In cultures where societal expectations and family pride exert strong influence, as in Thailand, parent feedback can unintentionally reflect these pressures rather than objective changes in a child’s abilities or behaviours. Parents’ hope for interventions to align their children more closely with neurotypical norms risks skewing their observations, making the results more a measure of parental expectation than program efficacy.
For researchers, credibility is paramount. Superficial claims of validity, without addressing fundamental methodological weaknesses, risk not only undermining the study’s impact but also eroding trust in the field as a whole. Autism research must strive for more than just appearing reliable; it must genuinely reflect the experiences and needs of autistic individuals. The ultimate goal is not to enforce societal expectations but to create interventions that are meaningful, supportive, and rooted in respect for neurodiversity. Only by prioritising the well-being and authenticity of autistic individuals can research truly contribute to understanding and improving their lives.
Reflecting on the Process: Why Critical Analysis Matters
Generalisability refers to the extent to which research findings can be applied to the broader population. In robust research, generalisability ensures that the results are not confined to the specific participants or conditions studied but hold relevance across diverse contexts and individuals. However, in the case of the ERMTP study, generalisability is fraught with issues—not just due to its design but also because of the underlying premise that the program’s aim is to teach autistic children to mask their authentic selves to conform to neurotypical expectations. From a PTMF and critical theory perspective, such an approach is deeply problematic, echoing historical patterns of coercion and conformity that have justified generations of systemic harm, including eugenic practices.
The ERMTP study’s small and homogenous sample poses the first obstacle to generalisability. With only five autistic children, all within a narrow age range and diagnostic profile (mild to moderate), the findings cannot represent the diversity of autistic experiences. Autistic individuals vary widely in their needs, strengths, and ways of perceiving the world. A sample so limited risks producing conclusions that reflect only a narrow subset of the population, whilst erasing those who don’t conform to the study’s criteria.
The study’s short intervention period—just two weeks—further undermines its relevance. Whilst pilot studies are often limited in scope, this duration is insufficient to capture the long-term effects or potential harms of a program like ERMTP. Given the program’s focus on teaching emotional recognition in ways that align with societal norms, the risk of long-term trauma for autistic participants cannot be ignored. Masking—suppressing authentic behaviours to appear more like a member of the neuro-majority—is not just exhausting; it has been linked to increased rates of anxiety, depression, burnout, and suicide. If extended use of ERMTP intensifies these pressures, the potential harm far outweighs any purported benefits.
From a critical theory lens, the very concept of generalising findings from such a study raises moral and ethical questions. Historically, interventions that seek to “normalise” marginalised groups have been used to justify oppression, from forced assimilation programs targeting Indigenous children to eugenic practices aimed at eradicating perceived “defects.” Whilst the ERMTP study operates on a much smaller scale, its premise—teaching autistic children to adapt to societal expectations—mirrors these historical patterns. It perpetuates the idea that autistic ways of perceiving and expressing emotion are inferior, needing correction rather than acceptance.
Looking forward, it’s easy to imagine how a larger, longer study with a more diverse sample could improve statistical validity and provide more representative data. But should we want this? A more generalisable ERMTP does not solve the ethical issues at its core. Scaling up a program designed to help autistic individuals mask their authentic selves only spreads the harm further. Instead of investing in research that reinforces the neuro-majority’s dominance, efforts should prioritise understanding and supporting autistic individuals as they are. Research should aim to dismantle the societal pressures that demand masking, not develop tools to help autistic people comply with them.
Ultimately, the ERMTP study reflects a broader issue in autism research: the tension between interventions that prioritise societal conformity and those that genuinely respect neurodiversity. True progress lies not in generalising programs like this but in shifting the focus to acceptance, accommodation, and the celebration of autistic authenticity.
